General Practitioner Telehealth Case Conferencing Clinic

Are you a GP looking for meaningful, flexible clinical work — one day in clinic, one day remote, without the full-time grind? Veteran Pathways & Proactive Care is seeking experienced General Practitioners to deliver chronic disease management and structured multidisciplinary case conferencing for veterans and patients with complex, multi-system health needs. The role runs across two structured days each week — an in-clinic day at our Ipswich partner practice and a remote telehealth case-conferencing day — with some flexibility on the days to suit you. This is not a standard locum. You will provide clinical leadership within integrated Chronic Care Teams, working alongside allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans. The work is structured, purposeful and genuinely multidisciplinary — a system that values thorough, evidence-based care over high-volume throughput. Remuneration First 3 Months (Guaranteed) = $240.00 per hour After 3 Months = 70% of billings Structured as an independent contractor arrangement.
